For patients, the service quality consists of any interaction with providers or delivery organizations for themselves or loved
ones. While there are many variables that can impact perceived quality, some directly relate to the gathering and retrieval
of information about a patient’s condition. Information collection and retrieval can impact time spent waiting or wasted through
repeated questions and information gathering processes as a patient interacts with different parts of a health-care organization.
This paper describes the MD:Notes project and its goals to improve information collection and retrieval that takes place in
physician-patient interactions in order to improve the quality of health care service encounters.
